Yes, exactly. What makes it more mysterious is that even the conventional prevention methods (never/quitting smoking, diet, exercise, etc.) aren't consistent nor completely reliable.

My condolences for your family's friend, and best wishes to your aunt, your other friend, and instructor for fighting the good fight. <3
