do you draw them by hand or using some kind of graphic program?

Both - I draw by hand on a graphic tablet.

im impressed by the quality of the graphics using that...i thought it would be more jagged.

It was hard to get used to at first, like keyhole surgery! You don't look at your hand or the pen, but up at the screen. But like most things, it gets easier with practice.
