Sam hadn't been feeling very well, so he had decided to take a nap in his room. He was so out of it, that he hadn't realized the door didn't close properly behind him. He lay on his back on top of the covers and stayed in all of his clothes.
The knock didn't register with Sam, but Mary's voice was slowly, very slowly, stirring him into consciousness. He groaned, moved his head back and forth and rubbed his eyes. When he stopped and opened his eyes, he saw Mary above him. It took a second - a brief second - to register what he was seeing. Once it did, deja vu washed right over him

Once the yelling reached his ears, it took John half a second to realize who it was. Instinct took care of the rest. He took off down the hall, his brain not taking a moment to wonder who 'her' could be, just set on being sure Sam was alright.
When he got to Sam's door, his brain locked up again, this time to shield itself against what it was seeing.
He couldn't see all of her, Sam leaning over her concealed her from the waist up. But John still recognized Mary immediately. "Sam?" He heard himself say it, but John's mind was continuing an automatic shut down, flat out denial dropping him into something of a daze. Something else was coming, a wave building and looming in his head, but he couldn't see it yet.
Sam didn't hear John. He didn't hear anything, not even his own sobs and constant repetition of, "No... no..." Just white noise.
It took a second before he could feel another presence there. He was half expecting it to be the yellow eyed demon, but he was too defeated to look threatening, so he slowly turned his head towards the visitor. When he registered that it was John, he broke down even more. He couldn't get out any words, so he just shook his head, assuming John would understand, and looked back down to Mary.

All he could see were his wife's legs.
