Punk:Attitude was on IFC last week late in the evening and I recorded it. This is probably one of the best documentaries on the subject, filling the supposed "blank" years from the demise of the Sex Pistols in '78 to the rise of Nirvana in '92. Surprisingly mentioned:

American Hardcore is out on DVD and it fills the same gaps you refered to. It's focus was on how Hardcore scenes developed locally in Urban areas across the country. They Start with LA and NYC and spend a good amount of time focused on Boston, DC, Minnapolis/St.Paul among others. My one beef with it is that when it speaks of Texas they really only talk about the Austin scene, namely MDC. There's no mention of the Butthole Surfers who are as important to the Texas scene as MDC is, and they spend no time talking about the nascent scenes in both Dallas and Houston (they do mention that Houston is where DRI comes from).
There's some great interviews with Ian MacAye, Joey Shithead, Keith Morris, Rollins, Vinnie Stigma, HR and Darrel Jennifer among several many others.

The Austin scene is missing from "Attitude" as well. I haven't thought about MDC in years. How could I forget those guys!
