I really, really enjoyed this. Will there be a follow-up? Because I can see this: He blinks and lets the world shift back into focus, waits for the hot spike of pain to shoot through his temple. Waits, but it doesn’t come. and they'll get used to it, maybe. And then one day it will come.

That's the way I felt reading this, like standing at the edge of the cliff, off-balance, ready to fall but ready to stumble backwards as well. Masterful.

Oh, my head hurts now. You certainly nailed the description of Sam's pain. And the guilt of being unable to go with Dean. Ominous but enjoyable!
