I don't know why they'd even ask her that. I thought the whole point of emergency medical attention was to try to keep the person alive! I may well be thinking about that tattoo also.
They asked that because some people don't want their hearts shocked back into beating if they go into cardiac arrest. It's best if you know the actual patient's wishes when family is not present. As long as she was still conscious, it was the best time to find out what she wanted them to do if she had another attack.
I suppose since they did not have a DNR on file they had to ask while she could still answer. Too many people are sue happy now so they cover their bases but I agree it was a strange question and a bit unsettling.
