I got dib when he was 5ish from a shelter, and he was just getting over being ill, so was underweight when we got him. he recovered fine, but has always been a bit thin and frail. Recently, he lost some weight, and i saw him paw at his mouth and he lost strength in his hind legs-all classic signs of insulinoma, so I took his blood glucose at home

Personally I would let him go... he seems like he's in really rough shape and I don't think you want to see his continuous deteriation. See how things go over the next couple days and judge from that.
Look at it this way though. If you lost the use of your legs... couldn't feed or drink yourself... and in a constant disassociative state wouldn't you want someone to help you be free of it?
It's really said that these type of things happen but such is life. At least you were able to rescue him and give him a fun and enjoyable life full of friends and love.
