if you make a blank layer under your stitches and go through with your regular brush set at a tiny round black point, and go fairly close to each "entrance" point for each stitch, then reduce the opacity of that layer to about 30% or so, it'll look more natural. Thread always pushes aside the weave in cloth to create a little hole, but they're never quite centered, so just zooming in close close on the piece and fairly slap-dashedly (a word? is now!) click click click through making holes will do, because nothing is ever perfect.
Here's one example (if you lower the opacity of the stitches themselves, don't forget to lower further the opacity of the holes). Here's another example at full opacity for stitches. It takes a while, yes, but I find it adds more realism to the piece as well as enhancing the depth of detail. If you're going to spend three hours on one piece, why not go a tiny extra step further, right?
