Not in Osaka, but I've done breast exams twice in Japanese-language hospitals. My hospital has a breast specialist doctor come in once a week. First they interview, then feel the lump, then a mammogram, then an ultrasound scan to confirm. The total ran me about 6000 each time. Finally they did a needle biopsy to check for cancer cells that took about 2 weeks.
