Okay here's another tutorial that was requested by
xxjaybabyxx Which flatters me to no end, by the way. Well I didn't save the original PDF file so this was trial and error until I got as close as possible so bear with me.
*Edit* I'm using PS CS2
Step 1: Get your base

http://www.gallery.davidandgillian.net/details.php?image_id=4864&sessionid=ca90435cbcfe158119d46e3eb4a4474b Step 2: Resize and crop; sharpen once and then blur Gillian's face with a small brush @ 22%. Then go back in with a small brush and sharpen her eyes and lips.
Should look like this:
Step 3: Make a new layer and flood fill with #e7dbc0. Set to Multiply at 100%.
Should look like this:
Step 4: Make a new layer and flood fill with #d4fffe. Set to Color Burn at 100%.
Should look like this:
Step 5: Now select all and copy merged. Open a composition and paste. Set to screen twice at 100% then floof fill with #08043b. Set to Exclusion at 100%. Then copy merge that and paste it back into your original composition. Sorry if that sounds complicated, but that's how I did it.
Step 6: Then set that layer to multiply at 100%.

Step 7: Now comes curves! Ha I can't believe I am acutally tutoring someone about curves because I really never use them much because they're so weird to explain but here it goes. Oh yeah sorry some of the caps are kinda small but you should be able to still read them I hope..
Go up to Image-->Adjustments-->Curves
Okay when it opens up this is the first screen
RGB
Move the line around until you get this.
This will lighten the icon up.
Then scroll down to RED
Put in these cordinates
This will add some red and some green to the icon
Then scroll down to GREEN
Put in these cordinates
This will add some magenta and some green to it.
Then scroll down to BLUE
Put in there cordinates
This will even out some of the tones.
It should look like this:
Step 10: (Last Step)
Make a new layer and flood fill it with #040229. Set to Exclusion at 100%.
