It's supposed to be a mirror shot (camera oriented behind him and showing reflection). Way not creepy enough yet. The doppleganger will look a whole lot nastier (peeling skin, rot and the like).
I think the most disturbing aspect is that you can't be sure if the... thing is really there at all.
