I believe Joss had decided very early on that either Xander or Willow would be gay, but hadn't decided which. If it had gone the other way, I suppose Larry thinking Xander was gay in Go Fish would be foreshadowing.
I don't know whether Anya knows what happened in the Wish. I thought that she didn't, but knew enough theory to have an idea what happened her amulet.

I love this episode, it's so funny. Looking back, season three is definitely my second favorite after season 4.
