Primary care providers in the city, outside of Callen-Lorde

Anyone have a great, trans-knowledgeable, primary care provider in the city that they'd like to recommend? I'm looking for a new PCP in Manhattan or Brooklyn, ideally one who is not at Callen-Lorde (CL is great, but will not work for me for several reasons).

Dr. Bartalos is in manhattan. I know a few guys have gone to him and had positive experiences.

There is also CK Life but that's in the Bronx.
