I've had an SPN theory for awhile but it would have been spoilery.

Nope. I was rewatching Malleus Maleficarum over the summer when the thought occurred.

Ruby at the end says: Yes, the same thing will happen to you. It might take centuries, but sooner or later, hell will burn away your humanity.

Every hell-Bound soul, everyone, turns into something else. Turns you into us, so, yeah...

Yeah, you can count on it.

Meaning how long it takes varies - Ruby still had yet to forget - but "everyone turns into something else"
*creepy shiver*

'cause of course if Ruby held out for centuries, we'd expect Dean to. Which is almost why I expect it NOT to have happened.

Hm. I don't know how I'd feel if that were the case, but they did possibly hint at it in Yellow Fever with the Dean is a dick/bully stuff.

There's even more in Kripke's statement re: Dean being a dick:

Victims contract the illness because they use "fear as a weapon." Dean asks Lilith at the episode's end, "why did I get infected?" And she cryptically responds, "you know why. Listen to your heart." We, as the writers, probably should have emphasized this mystery more, I take responsibility for that omission. But the point is: the reason he was infected is because of a SECRET he's keeping. A dark secret that will be revealed in Episode 10.

Which I had missed because I was equating Yellow Fever to Bloody Mary.
