Yes, he did leave Amelia (in the middle of the night no less), and he didn't know Dean was alive when he did it. Interesting.

There's certainly enough Dean guilt to go around, isn't there! I was seeing more anger from Dean at being seemingly forgotten or betrayed by Sam than guilt, but who knows.
If they leave Sam without any motivation, however, I am going to be an unhappy camper.
It was nice to see Sam stand up to Dean - it doesn't happen that often.
I don't think Sam knows. I think Crowley's powers have just gone up and up since we first met him. The weird hypercolours of Sam's flashbacks, while an interesting contrast to Dean's washed out ones on an aesthetic level, also give it an otherwordliness.
Dean was literally in another world. And I've heard it beign bounced around that a lot of the rosiness in Sam's flashbakcs might be implanted memories.
Maybe when the gate to purgatory broke open, so did whatever "spell" Sam was under and he woke at that moment to realize he was in bed with a stranger.
Like when Castiel came back and was all set up with a demon wife, courtesy of Crowley.
I'm just sayin, there's a lot of King of Hell in them thar hills.

Plus the whole "Sam you didn't try hard enough to save me" = "Dean you didn't try hard enough to save Cas" -- projecting a bit of guilt, perhaps?

Also, I think Sam left Amelia without knowing Dean was alive (sneaking off in the middle of the night - Oh Sam!).
I don't know why he was drove to Rufus' cabin afterwards.
I get the impression Dean had been trying to find Sam, but Sam was off the grid - they were both surprised to see each other.
