They find out that Blaine has early stage testicular cancer and that to be safe, they would remove a testicle (or both, up to the filler) and undergo chemotherapy treatment as a precautionary measure.
After the surgery/treatment and during the recovery, Blaine becomes very self-conscious and thinks that Kurt won't want to be with him anymore because he is either missing or has no testicles.
Kurt helps him overcome his self-consciousness and eventually Blaine feels comfortable enough to be intimate with Kurt again.
Please no self-harm or character death.

It first happens on their one year wedding anniversary, of all things. The rose petals that Blaine had sprinkled over the bed (that Kurt had protested were cheesy but secretly loved) have been pushed to the floor, the covers thrown back, articles of clothing haphazardly strewn. Kurt pushes Blaine back onto the pillows with a thud and a growl, mouth colliding with Blaine’s teeth clashing and tongues sliding together. Blaine groans beneath him, hands sliding up to clutch hard at Kurt’s hips, fingers digging into the soft skin as he ruts up.
Kurt smiles against his lips, kisses his way down Blaine’s neck, his chest, over his nipples, his belly, tight with anticipation.
“I love you so much,” Kurt hums as he moves further down, mouths at the crease of Blaine’s thigh, nuzzles the short hair at the base of his cock. He can hear Blaine’s breath stutter, feel Blaine’s muscles tightening.

“I can’t get comfortable,” Blaine complains, sticking out his lower lip.
“Why not? We cuddle all the time.”
Blush creeps into Blaine’s cheeks. “I’m… I’m still a little sore,” he admits and now it’s Kurt’s turn to frown.
