Why would a warden do this? Disrupting the status quo wouldn't get us anything. It's either an inmate with a grudge, or maybe an elaborate escape attempt, or something worse. I don't know. [ARGH HE HATES HAVING TO SAY THAT.]

Why would the other wardens do what they've done? None of them are perfect. An inmate would target everyone; a warden would target other wardens as a distraction. To do something.. bigger. "Worse," as you put it.
