Numerous have. There's a mountain with a temple, and a farm much further beyond; sometimes those who get punished are set to the latter. Still, looking out past the gates, there's only water as far as the eye can see.

Not literally. It's a spiritual world; it isn't a part of the afterlife, as some initially believe, but it also doesn't exist anywhere on earth ...or any of the other worlds people are from. Think of it like a crossroads.
