Major spoilers for Harry Potter, The Dresden Files: Blood Rites, Oliver Twist, Montmorency And The Assassins, Invisible Monsters. So be wary if you haven't read them and intend to at some point.
And I am totally not back-dating these because I was too sleepy the last couple of days.
Harry Potter Series by JK Rowling
There have been a few throughout the series that completely caught me off guard and you could have pushed me over with a feather the first time I read them; Quirrell being the real bad guy in Philosopher's Stone, Riddle's true identity in Chamber Of Secrets, Scabbers really being Pettigrew and evil in Prisoner Of Azkaban, Voldemort coming back in Goblet Of Fire and Snape killing Dumbledore in Half-Blood Prince being the main ones. This was the first series when I would have properly hurt anyone who spoiled it for me.

Montmorency And The Assassins by Eleanor Updale
Oh my goodness the ending completely took me by surprise and I completely dissolved and burst into tears. The opening of the second package, thinking it is one of the stolen exhibits only for it to be revealed as the body of Fox-Selwyn was absolutely heartbreaking, murdered by someone we thought was his friend and set up in a gruesome display. And this is a kids book! And he was my favourite character in the series. George!

Invisible Monsters by Chuck Palahniuk
So it turns out that the best friend is actually her brother who has become a girl to punish himself and that she actually shot herself so she wouldn't be beautiful any more or something. I still have no idea what the fuck was going on, it was really very confusing but it was very surprising whatever it was.
