They wanted to work Michael's rather nasty scar into the show. There is no point in writing in how that scar came to be unless the man with the scar is allowed to live in order to have a scar.
I don't see what the cause of panic is, personally. It just says one of the non-Tigh final fivers will have his/her life "hanging in the balance" as the episode's cliffhanger, and two other people will die. I can only think of one character, ever, who proceeded to actually die after a "will they die?" cliffhanger. (It was Natalie, and even that wasn't a typical 'cliffhanger' ending.)
Also, I'm bracing myself for characters I like to die, but two in one episode? Methinks they're defining the word "major" very loosely.
