Why does it seem that every guy I get into a relationship with ends up "not being ready for a relationship?" I've heard that shite twice in the past three years (which is not a good track record considering I've only had two relationships in the past three years), and both times I've been broken up with because apparently he suddenly realizes that

2 guys in 3 years is not a pattern, it's just a sucky coincidence. A really sucky coincidence.

If it has ANYTHING to do with you, it might be that you're attracted to less mature guys. Do you feel safer and more confident around someone who's inexperienced and/or boyish, etc? Maybe when you feel up to it you can experiment with dating new kinds of people. You're extremely attractive so you can probably take your pick.
