One of the new doctors in the psychiatric hospital, Alfred, falls in love with one of the patients: Ivan.
Or heck, I'll even take doctor!Ivan and mentally unstable!Alfred. As long as it has Russia/America in it.
And despite the title, you don't have to actually make one of them INSANE insane. Though props to you if you can pull it off. What Ivan or Alfred is suffering from, their circumstances, is entirely up to you as well.

“Yes! I know, but you are also nice to everyone, sincere to everyone, not just me. Physical affection means nothing to you!” Maybe it does to me. And Ivan was sure he had not said that out loud, but he still feared Dr. Jones could hear it being shouted and screeched in his mind, and his heart nearly stopped.
“Maybe it does to me, too.” Came the small reply, and suddenly Ivan felt as if he wasn’t the only one who belonged here.
The silence was nearly deafening. It hung between them like a strand of lights, blinking meekly.
Ivan heard the doctor shift but paid no attention to it until he felt lips on his cheek.
Doctor Jones stood up from the bed and walked towards the door. His hand froze over the sterilized knob as Ivan began to speak, and for the first time in two years Alfred heard his given name spoken in the white washed sanitarium.
“Thank you, Alfred.”

A/N: Ivan is pretty much diagnosed with Schizoid Personality Disorder, but I think it would have to be fairly severe if he was actually placed

Well maybe because SPD patients can suffer of clinical depression, anyway this was actually really sweet anon and.....hehe Alfred must look good on a doctor's attire.

Luckily for him, but not for the staff of that unit, they hadn’t started morning report yet thanks to an emergency on one of the wards. Not the forensic ward, luckily. Which was his ward. One of them was his anyway. The thought made him nervous, but it was sort of overshadowed by a growing excitement threatening to burst out of him like, like an alien or something. Holy crap! Forensic psych! Fucking awesome

omg, anon, this is sounding like it's going to be EPIC! I love how many characters (Monaco!) you've been able to work in, and the mystery you're creating around Ivan. Can't wait for more!

Finding ways to work in as many characters as possible was part of the fun! And, I'm really glad you like it, anon! I'm actually not very confident in my fic-writing ability, especially for Hetalia, so knowing even one person liked this is very encouraging ♥
