America and England are in a caring, loving relationship. However America just doesn't know his own strength and during sex tends to accidently hurt England, leaving bruises and making the entire experience quite painful. England however feels he can't mention it ( due to pride, worry that it might impact their relationship, worry America will feel guilty or get too concerned touching him, not wanting to appear delicate etc.). Another nation, or America himself notices the bruises on England, coerced him into admitting the problem, and tells America, who freaks out.
Bonus: England goes to great extents trying to hide any bruising/ injury.
Bonus: After it's resolved they have gentle, loving sex( Or you could just allude to it :)).

At first though he was reluctant, England was pressured to tell Poland (and a nervous Lithuania) how this all came to be and why America had yet to know. When Poland asked how America formed the bruises, England blushed and looked away and Lithuania instantly knew ( ... )
