Good for LJ venting, and good on you for sticking with the so far.
I know this may not sound like it will help when you have so much to do but you need to give yourself at least some small breaks. Think of it as sustainable studying.... by having a few breaks you actually will get more done in the long run.
Personally, what works for me in these situations, that probably doesn't work at all for other people, is every now and then, just go out and rock. A single good night out dancing or partying or listening to great music doesn't just take your mind off it, it leads to a good nights sleep, and a burst of renewed energy, burning off weeks of anxiety. This might now work for you in the least. But it's how I get through those periods. Other people try meditation, etc. But remember it is OK to take some time out of your schedule to deal with anxiety how ever you can if it is going to reduce anxiety and increase productivity the rest of the time.
See if you can talk to one of the councilors at student services. They were really good with helping me get on track during my honors year when I came out of hospital and had heaps of study to do.
I agree wholeheartedly with anxiolytic, doctor_k_ and strangedave. Realistic planning, a get-shit-done attitude and a sufficient amount of stress-relieving activity are all very important.
Anyway, hang in there and keep at it. Between evenings and weekends, 20+ hrs a week is totally achievable.

Anyway, I have confidence that you can do it.
