At my company, we have to keep a really well-documented paper trail before someone gets fired. We had one girl working here about 6 years ago from India - she had a masters degree from a respected university in computer engineering, an awesome GPA, had been a TA for several years, had sterling recommendations, and a stellar resume. She was solid gold on paper. Unfortunately, she was the most useless excuse for a programmer I've ever even HEARD of. One day I shall list her myriad fuckups, and no one will believe I'm being serious.
Anyways, we had a hard time getting rid of her. We had to show that she had been counselled, given a bunch of opportunities, and that complaints had been lodged from multiple people. We STILL couldn't fire her directly; the worst we could do was to refuse to let her work on our projects. She had to find a different project to charge to on her own, and when she couldn't find work, she was de facto fired
