I'm pretty "new" around here (hi everyone!), but already I'm chomping at the bit to fill a lot of what everyone's been prompting! Couldn't hurt to put a few of my own up... ;)
Wish 1. Through a hex, spell, or what-have-you, Sam and Dean are linked in such a way that whenever Sam is injured or sick, the illness or injury is automatically transferred to Dean. (Sam gets shot on a hunt? The bullet wound materializes on Dean's body instead. Sam tests positive for mono? Dean has all the symptoms). Cue worried!Sam trying to figure out what's going on and why.
Wish 2. Outsider POV (probably a nurse). Dean's just been brought into the ER in pretty bad shape - bad enough to need an ambulance. He's probably delirious, super weak, in shock from blood loss, you name it. When EMTs wheel him to triage he's limp as a noodle - but as soon as the nurses try to intubate him or cut his clothes off, his survival instincts kick in and he fights like an animal to get out of the hospital. (Maybe he left Sam in need, or he's delirious and thinks he's in

Oo thanksss! Really I was just trying to think of a way to create more food poisoning sick fics without Dean actually being sick with food poisoning. So.
And also, the idea of Dean saving people over and over no matter how badly it hurts him makes so much sense to me.

I was sitting there going like this O.O for entirely too long re: #3. I'm always sort of sad that there aren't more Mystery Spot fics (that I've found anyways) dealing with the fact that a lot of those deaths were not instantaneous and were, in fact, horrible (death by fucking golden retriever?). I'd always imagined it as Sam helping Dean through death after death but your idea is so much more devious better.

Yes! I'm completely with you re:Mystery Spot. Watching the ep, half of me was like "this is hilarious" and the other half was like "but what if Dean remembers everything and he's singing Asia in the morning to try to cheer himself up before he dies, and maybe if he just keeps acting like nothing weird is happening Sam won't feel so bad about his deaths" and ahhhhsdflkjd.
