Wow, this is stunning! His expression is so very Dean and I love how he's clutching the gun and holding his head. It reminded me of the scene when Dean shot the youngest Steyne.

I'm also stunned by this amazing Dean ouwies piece!
He's so hurt, reeling, and either just pulling back from a fight, or trying to get back in. The disorientation is palpable. His gun in his left hand, yikes. That gives me the idea that he's been knocked for a loop, gotten back up and either trying to find his way home, or gathering himself for round two. Is he needing to rescue Sam? Oooh, it's so evoking!
