I'm pretty sure I know what you're talking about... is it possible Dean wasn't mentally ill, but had a traumatic brain injury that made him behave in a childlike manner? John and Sam (Sam is college-aged) have abandoned him for several years in a care home, but John hears that something might be after him so he and Sam both go to get him out. Then they go back to Kansas and visit Missouri Mosely. They find out the care home has burned down behind them. There is something haunting Dean, like a ghost or something. There's a scene where Dean leaves by himself to go on a walk and ends up at a park where some teenagers see him and start harassing him after realizing he's mentally disabled. But the dark thing that haunts him protects him and attacks the kids before Sam finally finds him and takes him home.

The fic is called "The Innocent" by ysbail, but their journal is purged. The fic was also never finished. I do have a pdf I could share if you think it's the one you're thinking of.
