Done with CS2 and very simple.
1.
Prepare your base. The advertising movie still just needs cropping to 100x100. Make a new layer. Select your eyedropper tool, pick up the colour on Ennis' jacket and using a 9px round brush, cover up any remaining visible text. Merge layers and desaturate. Duplicate twice. Set first copy to screen and the second to soft light. You should now have this;

Then select this gradient
and paste in on a new layer. Set to soft light. Duplicate the gradient layer and you have this:

Take this border (sorry, I cannot remember who made this - can someone tell me?);
and stamp it on in #C0A180. Either set to screen to lighten the edges or leave as is. And finally some text. The words are from the movie ad, the font is Times New Roman 4pt, #E1D2AG, kerning 150, AA sharp and stroked with 1px in #0F0902. And that's it.
