Today I'm going to be using Jensen Ackles as my model.
1. Take your image, crop/resize it to 100x100 pixels. Sharpen, smooth, whatever your preference is.
2. Now depending how dark your image is, you might want to duplicate your base and set it to screen. If it is light enough for you skip this step completely. If you chose to do this, merge all layers when you are done.
3. Create a new raster layer and set it to Multiply, 100 opacity. Flood fill with #f4be68.
4. Create another new raster layer and set it to Soft Light, 100 opacity. Flood fill with #ebc7c7.
5. Create yet another new raster layer and set it to Burn, 100 opacity. Flood fill with #81a7d2.
6. Create a final new raster layer and set it to Exclusion, 100 opacity. Flood fill with #07132d.
7. Duplicate your base and drag it to the top. Set it to Soft Light.
8. Now create a new raster layer and set it to Dodge, 100 opacity. Flood fill with #07132d.

If you used the screen layer and set it to 50 opacity you get:
