Sorry if this is terrible, I've never really tried this sort of thing before.
01. Crop your image to 100x100.
02. Now I messed with my hue/saturation/lightness, but that was because I wasn't satisfied with how it was in the original image. So that's optional. But in case you were wondering, I set the hue to 9, saturation 37, and lightness 14. Same for brightness/contrast. My brightness is 10, and contrast is 21.
03. Duplicate the base and set it to Soft Light, opacity 100%. Merge the layers.
04.Create a new layer. Flood it with #214A74 and set it to Hard Light, opacity 70%. Merge the layers.
05. Paste this gradient by
_gillymaniac
and set it to Darken, opacity 100%. Merge the layers.
06. Set the brightness to +10, and the contrast to +20.
And you're done!
