is there something i'm not getting? why is fresh pepper followed by a question mark and why is it not coupled with something else?

my take on that is that it's a reference to the Op-Ed piece that it accompanies, in which the author talks about having trouble making decisions when faced with choices (including scenarios where he is at a restaurant and can't decide what he's going to eat....) So, it is quoting a waiter asking if he would like fresh pepper, and... well... does he?
