I'm curious, at the beginning of the series because Ryoma knows about Momo's ankle, does he also know about Tezuka's shoulder/elbow?

Nope. I think only the seniors knew what happened, and only Oishi and Fuji knew (or maybe just Oishi?) that it was still bothering Tezuka.

I think he did, he's a genius after all. I think he noticed something was wrong but he couldn't... pinpointed it

I tend to think not. Ryoma is perceptive, but Tezuka is pretty inscrutable, and he hadn't had to play at a level that would expose his injury until Atobe.
