Also, we don't use wrist or shoulder weights when playing. For one, it screws with your timing. Second, they add stress to the joints. We concentrate on strength building during our normal workouts instead.

Sanada-san, especially, works at high speed with those pitching machines. That puts a huge amount of stress on the shoulders.
Painkillers don't help much once you blow a shoulder.
