Sorry to hear about all this, Jeff! The US medical industry really bites. I was recently insured for a year as a benefit of my bakery job. The doctors I saw were actually LESS good than the ones I'd been seeing at the public clinic while uninsured. And I still ended up paying huge "deductibles" for a service I had supposedly already paid for.

Anyway, it sounds horrible that you're in pain and your insurance company is denying you coverage. Maybe you should write a graphic novel about your plight and have each of your artists pitch in to draw a chapter? Blow the lid off this thing.
