I signed up to volunteer at the library by helping with shelving yesterday. I just received an e-mail telling me that all volunteers must go through a formal screening process and that "a minimum of two references may be contacted." They also will contact me to discuss my interests, talents, and the amount of time I wish to volunteer

Try emailing them back and explain that because of your disabilities, you need to have the "interview" conducted through email (text) because you have physical difficulties (might even say outright that it's CAPD) with using the phone... If you have any kind of note from your doctor, offer to provide it.

I'd say teachers is probably the best for references... Even one whose class you aren't in anymore, since they would also know you despite the information being slightly outdated.
