I guess since we don't know how long John was actually with her when they first met, it's hard to tell.
As far as the 2nd question - well, Dean knew YED was planning something big. In Bobby's words "End of the world big'. So, it wasn't like he was going to bring Sam to a bright and shiny new world with no demons. Not to mention that he was bringing Sam back to whatever 'destiny' Sam had which Dean had no idea what it entailed but it could have ended up with him having to kill Sam and most importantly, Sam having to burry Dean and live without him. So, I don't see how the world Dean brought Sam back into is much better than this one.

Dean's definitely not feeling up to the task. But honestly, as far as bringing a brother back from the dead, with Sam he couldn't think of anything else. He couldn't live without Sam. Adam was someone he really never even met.
It's hard to know, especially not knowing how long they were together initially.
I thought about Tessa the first time I saw that scene. That was my immediate reaction. But on the 2nd viewing I started thinking how this is different from when Sam died? I mean, Sam might have been in a better place, but Dean chose to pull him back. And if that's the only reason Dean said it, then why not try what Sam proposed and ask Cas for a favor?
Really? I felt like Sam didn't even flinch and Dean was the one who was actually taken aback by Sam actually admitting to it and liking that comparison. I think Dean expected Sam to be all 'I'm not like him'.
That scene at the end, well I think I mentioned it before, but I wasn't sure HOW to take Dean's last 'take it anyway you want'. But as far as Sam, I do truly believe that Sam now completely agrees with what John was trying to drill into him his entire life. Sam lost any hope of ever leading a normal life. Hunting is the only thing for him and making sure that he and Dean survive what's coming. He wants revenge and he wants to fight. Revenge was a BIG theme in that episode. And it was there for a reason, though honestly Kripke really doesn't need to treat us like idiots. We all know what's driving Sam. We don't need the word 'revenge' repeated a bazillion times to get it

#1 - I think that yours is the reaction the writers were hoping for, and the same one that Dean had - like his father isn't the man he thought he was. John Winchester has always been held up as Dean's role model, and a sort of tragic, mythic hero. I actually think this makes him more human, and I like it. She probably kept it going in part because of Adam, and since it sounds like she knew what John did, then she understood.
#2 I think Dean brought Sam back because he was selfish - he couldn't make it without Sam. Plus, John had hammered it into his head that it was his job to take care of Sam, and he feels he failed somehow. As far as what Sam remembers, it sounds like he doesn't remember anything. Maybe he was in some kind of purgatory or limbo, and since he wasn't dead very long, he just doesn't remember, so it doesn't occur to him that Dean might be right.
#1 I agree. We were supposed to see the whole thing from Dean's point of view. It's interesting that Sam completely accepted the fact that Adam could absolutely be their brother, that John wasn't a monk. But then Sam always saw John's faults, even when he was a kid. He didn't idolize John the way Dean did.

I personally choose to believe that Sam was 'in a better place.' Because honestly who deserves it more than the Winchesters? And at that time when Sam died, he didn't even make the choice to go darkside yet.
1. I think the mother probably just tried to have a friendship with John so that John would feel comfortable and welcome enough to come back and see Adam,she could have though if she ignored him and hostile to him she would scare him away, and she seems to love Adam maybe? idk!
2. No he so never did even consider where Sam might be, Dean cant live without Sam thats pretty much all he could think, I wish we knew where Sam actually was
As for #2, obviously Dean only thought about not being able to live without Sam. I just wonder if he even paused for a second to think about where Sam was. Even if it was after the fact. Same with Sam. And yes, I would love to know where he was. And also I would like to actually have Sam say something to Dean about it.
