80yoM Pt entered ED at 703 complaining of Dyspnea x 20 hrs. Patient states he was "mowing his yard yesterday afternoon" around 1600. After completion PT 'could not catch his breath'. Pt states discomfort was mild, and not relieved by position. PT states he 'smokes 1 pack a day'. GCS = 15, A&O times 4.PTs responses are lethargic. Neg speech
