They do look at the camera. They do talk to the people following them around, and chat with producers. But they handily edit out those parts. And for your other point, how is having a camera with a big ass light on it any more of a distraction than knowing that your parents and friends are being driven away due to your self-destructive behavior?

The horrible thing about addiction is that it burns bridges at a time when human contact is what's needed the most. When your own family won't talk to you and you don't have anyone to tell you it's okay, why should you care if somebody's pointing a camera at you or not? Everybody else has made their decision about you, what's one more gonna matter?

That's a good point: no reason to be self-conscious at the bottom.
