Making garb look like clothes does take practice. Knowing where what ought to drape and fall can take a little research, and knowing how much extra to add when the instructions add way too much can be problematic. What are you making? (Besides the skirt--which I'm guessing is a dirndl...?)
Is Surplus City down near Altoona still open? Their fabric selection was hit-and-miss, but the hits were great hits.
As for chirurgeoning, I'd had CPR and first aid certifications while I lived in Nithgaard, but never bothered with the office cuz I got stuck as chatelaine and herald!
I'm making a rubakha (Russian T-tunic, full-length) and a panova- the skirt (I was too vague in my post: this is three rectangles of fabric hanging lengthwise from the waist long enough to reach mid-calf or ankle, and wide enough together to reach around the waist. The fabric used for these was often checked or plaid. I still need to double-check what colors would have been available.) I need a head covering, too. The Russian povoinik is a cap worn by married/widowed women that looks similar to a St. Birgitta's cap, but also kind of different. And I've seen sketches of different styles.

Chatelaine AND herald? Oy. We recently elected a new chatelaine, and Aindreas has been serving as herald and fencing marshal since before I joined. There's also this weird undercurrent of Shire politics that no one seems to talk about, but it's still obviously there, and I don't grok it at all.
Hm. I don't know much about those garments, but one of our local-isch friends, Scholastica la souriete, has a book of Russian embroidery and lace that has that rubakha pictured in. She's also hoping to part with it, though I don't know how much (just that it was expensive when she got it online and she normally sells at cost). I could inquire if you're interested in primary sources at all.
I don't remember having both offices at once, but yeah... Things got weird when Phiala got her Laurel, and she started campaigning to get Thorvaald a shinycookie as well. There was also normally a rift between the Meduseld crowd and everyone else because of Aurora snarking at a few people, but I don't know if that's gotten any better.
