Didn't they reduce the number of userpics on most account types in the past couple of weeks?

Perhaps, but I was below cap. Also, the missing icons still show up as options. They just come up without images. It is strange.

The icon server seems to choke and burn a fair bit lately(enough that I notice it on the rare occasions that I visit LJ). So it could resolve itself in a couple of hours/days.

The one that I first noticed missing is an icon I created from a photo I took. Good thought, though.
