Paragraph fixy thing:
In Word.
At lower right, there's a little circle button (above and below it are double arrows pointing away). Click it. Then there's a box with tabs. Pick "find and replace."

Put "^p" in the first box and "^p^p" in the second box. Hit "replace all" and you're golden.

You can do other things that copy badly, too: "^+"/"-- " for em-dashes (I usually do ^+" to --" first so I don't get -- " in dialogue), uncheck "replace straight quotes with smart quotes" and do search and replace " to " and ' to '

To do italics, you have to do it partly manually: go to the "find" tab, clear the box, hit ctrl+i, and that'll find italic text (ctrl+pagedown to go to the next one) so you can HTMLify it.
