It didn't strike me as more raunchy, although maybe a little. It has more gore, and a bit more self-awareness at its own silliness... and it's more gothy.
The plot really is put together like one from a dramatic opera, but the whole thing's set in a society where there's massive amounts of organ failures. One company rose to fill the need for organs, and is able to offer payment plans if a person can't pay for it right away.... and they also got legislation passed to make organ repossessions legal. So they have repo men... for organs.
