Maybe you should ask the doctor you work with how he feels about the sad parts. I had big problems with physics research once, because I was worried that anything we learn in nuclear physics will contribute to weapons technology. I asked my boss about how he reconciles that for himself. They may not have been the right ones, but he had answers.
i also think that you'd be a wonderful neurologist. every day things that we thought were fact are being proven wrong... the lab i work in is working to find out what exactly goes wrong in heart scarring (and my very own project is growing replacement heart tissue)and more important (to you at least :) the lab next door to mine is working to develop protocol for regenerating spinal cords after they'v been damaged, and they're making alot of progress (dr. shelly sakiyama-elbert, if you're interested) stroke damage can be greatly reduced if the patient is treated quickly, so greater awareness and better management of the hospital's emergency services can reduce the damage from that

Sometimes people get sick. Sometimes they can get better and sometimes they can't. Any if it were my loved one who had no chance of improving, I'd still want the best doctors. You might not be able to bring me joy in the way that you could save my loved one; but you'd bring me joy in being helpful and knowledgable and compassionate. In that you'd care for my loved one and try everything you could.
Sometimes you can't change disease, but you can still bring joy in how the disease is dealt with. Someone's gotta do it, and I know I'd love it to be you.
I say don't be too afraid. There's always room for change.
