I don't think Ica is stupid to think she's the only one to feel the way she does. She doesn't have anyone to talk to, she doesn't have anyone she feels she can connect with or be with. It's true, that she's not alone, but that fact doesn't matter because she feels that way, and even knowing she's not alone, isn't going to help much.
In the end, Ica died. But is that the ending she wanted to her story? Is that just what her life lead to? She was alone, she felt so alone that her heart hurt in her chest, and it literally felt like it was going to burst in her chest. Why is it she doesn't feel like her life will lead to anything and that only when she dies, she'll be free? What if after she dies, she really isn't free, but instead, a prisoner of her lonliness forever? What's the ending that Ica wants, and why can't she have it?
