**You MUST be 18 with a VALID government issued PHOTO ID to be pierced. If under 18 you must have your Parent or Legal Guardian present to sign for you. You must have a photo ID (school ID, military ID etc) your parent or guardian must have one as well. If for any reason your last names are different I will need to see a birth certificate. If your legal guardian is signing for you I will need to see the court issued paperwork showing guardianship. NO ONE UNDER THE AGE OF 14 WILL BE PIERCED!!!**
