What you need is called your "Statement of Earnings" or 源泉徴収表,
assuming you mean the little paper in the picture on http://www.hyogoajet.net/wiki/Taxes_Back_Home . My former office is sending mine to me as a pdf, so you might even be able to get it via e-mail!
