"Geek Love" by Katherine Dunn.
The title refers to "1 : a carnival performer often billed as a wild man whose act usually includes biting the head off a live chicken or snake" (definition courtesy Miriam-Webster)
The main premise of the book is that a husband-and-wife team decide to revive flagging public interest in the carnival they run by expanding their freak show. They do this by experimenting with drugs and chemicals and having a series of children who are born differently and uniquely formed.
That is the most normal and acceptable part of the book. This was the sickest, most twisted, most fascinating, most horrifying book I have ever read.
