When I am supreme ruler of the universe, this will be one of the first laws passed:

If a book is not the first book in a series, said book MUST have printed somewhere on the front or back cover the fact that it is the ___ book in a series, and one of the first pages inside must list the books of the series in order.  (Unless it's like Discworld, ( Read more... )

There should be no text before the story begins that gives away any plot points. I don't care if it's labeled "introduction" or is an introduction cleverly labeled as a "prologue" - or anything else for that matter.

I'm looking at you, current publishers of "Day of the Triffids".
