This sort of plot bunny has been used in published fiction before, though not as a focus for a vengeful ghost that manifests outside the recipient's body. I think usually it's more like a possession, with the spirit of the donor taking over the recipient.

I can see S&D arguing this one out, and I don't know who would win... or maybe an exorcism would do the trick.

Transplants aren't thought of as being as scary as they used to be - a book I read WAY back in the 70's, Cold Cuts I believe it was called, there's a murderer who wills all his body parts (even his brain, which struck me as monumentally stupid) for transplant after his execution, and then goes about reassembling himself with the aid of a mad transplant doctor. Bwa-ha-haha-ha!
